For your convenience and to save you time at the office, please fill out any forms that you’ve been instructed to:

  • CROFT Welcome form/Confidentiality and Consent Form
  • For Children and Teenagers: Parental Consent on Behalf of a Minor
  • Consent to Release Confidential Information

These forms are for new clients and cover intake, consent, and confidentiality. Please review them carefully before filling them out.

**Should you book an appointment through the website, there is an additional form to fill out that is available for download after you have submitted your request for appointment.**
